Dr Lily Tomas prioritises her ongoing patients to maintain their Optimal Health and Wellbeing. Lily opens her books periodically where -ever possible to cater to the needs of New Patients, however this is dependent upon current availability.

For all appointments for Greg de Jong including Physiotherapy, Scenar and HeartMath please call 0264958413 and make a time with our support team.
Referrals are not generally required for appointments unless under specific programmes.
If you have an Enhanced Primary Care (EPC) Plan / Chronic Disease Management (CDM) Plan please bring your referral form from your GP to your first appointment and a Medicare card to allow processing of the Medicare rebate.If you are seeking health care rebates via HICAPS please bring your Health Fund card to the appointment.
Greg is currently not seeing Worker’s Compensation clients.
New Patient Information for Appointment
All appointments are generally 40 minutes, however if you have a complex or long-standing issue you may consider a Long Consultation of 60 minutes for an extended evaluation of your ongoing needs.
On making a New Patient appointment you will be sent a link to a New Patient Questionnaire appropriate to your condition via email using the Ciniko system.
Please fill in this form in as much detail as possible as this will allow for more time for treatment at your initial assessment. If you are unable to fill out the form before your appointment please attend 15 minutes early to your New Patient Appointment so you can complete the new patient questionnaire onsite in as much detail as possible.
Please consider wearing clothing that allows for access to any area you are requiring treatment for and that which retains your comfort. Disposable gowns will be made available where required.
